Nau mai haere mai! Welcome to your Board and/or Trust Health Check.
Great governance doesn’t happen by accident : it takes regular reflection and action. This tool is designed to help Board Directors and Trustees assess their governance effectiveness based on key drivers identified through research DTK and Associates has published.
By completing this health check, you’ll get a clearer picture of what’s working well and what needs attention, so you can ensure your Board or Trust is on the right track.
Choose which format you would like your quiz participants to use
- PDF – We have created a PDF version of the quiz so you can send participants a PDF to complete the quiz with a PDF viewer of their choosing or print and complete it with pen (and scan to send their response back). Surveyors can manually compile results.
- Microsoft Form – We have created a Microsoft Form template for you to copy and use to have participants complete the quiz online. Surveyors can view results in an Excel table or in the responses overview summary section. Be sure to duplicate the template first.
- Send out the quiz in your chosen format. We recommend using Microsoft Forms as results are automatically complied, removing the need to enter data with the PDF responses manually.
How to complete the quiz
- You will answer 31 questions, grouped into three sections.
- Each question presents a governance statement—simply indicate whether you agree or disagree.
- Answer honestly—this isn’t a test, it’s a tool to help you reflect and improve!
What to do next
- Choose how you want to complete this check: online or in a written format—whatever suits you best.
- Once finished, review your responses and identify strengths and areas for improvement.
- Discuss your results with your colleague board directors and/or trustees: great governance is a team effort!

How to use the Microsoft Form

Open the template link
- Click on the link I provided to access the Microsoft Forms template.
- The form will open in your browser (no download needed).
Make a copy of the form
- Click duplicate it at the top of the form.
- This will create a copy in your Microsoft Forms account.
Edit the form (optional)
- Go to Microsoft Forms (https://forms.office.com).
- Open your copied form and make any necessary changes, such as editing questions or adding options.
- Delete the warning about copying the template piece.
Share the form with others
- Click collect responses at the top of the form.
- Choose how you want to share it (link, QR code, email, or embedding).
- Adjust settings like who can respond (anyone or only people in your organisation – this will depend on your Microsoft account and your needs).
View responses
- Go to your form and click responses to see results.
- Click open in Excel to export responses for analysis.
